Healthy Aging Projects Supported

The mission of the Tufts Health Plan Foundation is to improve the health and wellness of the diverse communities it serves, with a focus on healthy aging. At the core of this work is the belief that older people are a critical asset to building and sustaining vibrant and healthy communities.

The foundation currently is inviting applications for its Momentum Fund, a mini-grant program designed to build on the region’s energy, experiences, and insights to create cities and towns that are great places to grow up and grow old. Through the program, grants of up to $10,000 will be awarded to community-led projects in Connecticut, Massachusetts, New Hampshire, and Rhode Island that promote healthy-aging solutions for older people. Projects may include cross-cutting determinants that influence active aging, including the built environment, transportation, and employment and social inclusion.

Preference will be given to projects that make a community more age-friendly/dementia-friendly; advance promising solutions prioritized by the community; feature collaboration; and address inequities. The foundation values projects that address the needs of diverse populations, including racial or ethnic groups; women; immigrants and/or refugees; those with low income, disabilities, mental health conditions, or limited English proficiency; those who identify as LGBTQ; or those who live in rural communities.

A webinar info session about the opportunity will be held August 22 from 2:00 p.m. - 3:00 p.m. EDT.

To be eligible, applicants must be  tax exempt under section 501(c)(3) of the Internal Revenue Code or have a fiscal sponsor with such status.

See the Tufts Health Plan Foundation website for complete program guidelines, application instructions, a list of previously supported projects, and to register for the webinar.
