Support for Initiatives Addressing Diabetes and Hypertension

The American Medical Association Foundation (AMAF) announces the availability of funds through our Community Health Program. AMAF seeks to financially support innovative programs targeting the prevention and reduction of type 2 diabetes and hypertension. Grant funding will support community-based organizations such as free health clinics that provide services to vulnerable and underserved populations.  

The AMA Foundation aims to increase health equity and improve health outcomes among vulnerable populations utilizing evidence-based models of care. Committed to fostering focused and measurable health improvements, Community Health Grants will be awarded in amounts up to $50K and are renewable for up to three years.


Organizations must submit a letter of intent to begin the process. The application portal will open on November 20, 2019 and close on January 17, 2020. All eligible organizations are invited to submit their Letter of Intent prior to January 17, 2020 at 11:59pm CST. In order to gain access to the application portal, the organization representative must complete the pre-submittal eligibility quiz. If deemed eligible, the organization representative will be able to complete the profile and submit their Letter of Intent.  Submission of the Letter of Intent does NOT guarantee an invitation to submit a complete application.

Organizations approved to submit a complete application will be notified by February 11, 2020. The complete application must be submitted by March 20, 2020. 

This grant fund opportunity is open to organizations that meet the following criteria:

  • U.S based Non-Profit Organization with 501(c)3 status*
  • In operation for over 3 years
  • Annual organization operating budget less than $3 million
  • Provide services in the continental U.S.

*Organizations are not allowed to utilize third party foundations and/or fiscal agents to submit applications. This includes, but is not limited to, university and hospital foundations. Organizations with multiple sites are NOT allowed to submit for an individual site. In addition, the following organizations are ineligible to apply:

  • Medical student-led clinics
  • Hospitals
  • Universities
  • Elementary and high schools
  • State and Specialty Medical Societies
  • Local/City/State Government Entities
  • *National affiliated organizations (utilizing the same EIN/501(c)3 status)

Pre-Submittal Webinars will be provided for those organizations selected to submit a complete application. These webinars will offer a comprehensive overview of the Community Health Grants program and review the RFP timeline. AMAF Program staff will also address any questions and concerns presented by webinar participants.

Attendance is not mandatory but is strongly advised. Interested respondents need only register for one webinar as they will contain the same content. Those unable to attend will be able to access the completed webinar presentation via the AMAF website after the first webinar has been completed.
